All new patients are asked to PROVIDE PHOTOGRAPHIC ID AS PROOF OF IDENTITY & PROOF OF ADDRESS and complete a registration form and new patient questionnaire. You do not need to make an appointment unless you need or wish to be seen by a doctor. Unfortunately, appointments cannot be made without being a registered patient, either temporary or permanent. Once the registration form has been received, any urgent health needs can be dealt with by the on-call GP. Regular appointments can be made after the registration has been processed and accepted
Pre-registration - If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the 'Send' button to mail your form to us. Please visit the surgery within 14 days to sign the form to confirm that the details are correct and provide proof of identity and address. You will not be registered at this Practice until your details have been confirmed.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
We will not discriminate against anyone if identity documentation is not available, registration will still be completed.
